Description: Jefferson County Public Health is currently hiring an Advance Practice Provider (APP) in our Sexual Health Clinic. This direct-care position involves clinical public health work to promote health of individuals and impact communicable disease rates and improve access to high quality medical care. JCPH provides comprehensive Title X services, testing, management, and prevention of Sexually Transmitted Infections (STIs) and HIV to diverse patient populations
This position is within the JCPH Clinic Services Division which provides immunizations, sexual health, and harm reduction services to our community. This position must be comfortable seeing gender diverse patients both in a clinic setting and conducting outreach with community partners, including homeless centers and correctional facilities. Works as part of a team to deliver high-quality medical care.
This position works >30 hours/week with flexibility in establishing a set schedule, falling on Monday-Friday, 0700-1730. This position requires onsite work to be performed at our Lakewood Clinic location and at community/outreach clinics.
Anticipated Hire Range: $102,000 - $120,000 USD Annual (based on a 1.0 FTE status) Compensation will be determined based on FTE, education, experience, and skills.
Limited Benefits: The employment status is limited (Fixed-Term) with an anticipated assignment end date 3-6 months after the hire date with the possibility for an extension or move to full employment status with all Jefferson County benefits. This position is eligible for medical coverage starting the month after hire.
Essential Duties: - Provide evidence-based, top-quality care to a diverse population.
- Adhere to the principles and mission of Jefferson County Public Health and federal and state programming.
- Follow designated guidelines and division and program policies and protocols provision of care, including in assessment, diagnosing and treatment/management of medical conditions and preventive care. Consultation with attending providers as needed.
- Determines and assesses the health status of patients by reviewing records, taking comprehensive medical histories, and performing physical examinations.
- Initiates, performs, or directs other healthcare professionals to perform screenings and diagnostic tests.
- Monitors and evaluates medical condition of patient.
- Makes decisions regarding treatment changes and refers patient to specialized services as needed.
- Orders and administers appropriate medications.
- Completes required EHR documentation thoroughly, and in a timely manner. Participates in clinical education of healthcare students as assigned.
- Involvement in clinical coordination and special projects, including administrative tasks as Assigned.
- Attends and participates in program, division and Department meetings.
- Utilizes the principles and practices of effective communication to elicit information, negotiate problem resolution, and/or garner support for various programs or policies.
- Follows established departmental policies and procedures and adheres to any quality improvement, safety, environmental and infection control standards.
- Perform procedures, including but not limited to, the insertion and removal of IUD's and contraceptive implants, cryosurgery, colposcopy. Conducts clinical activities as defined in the privileging documents for this provider.
- Additional medical service lines may be developed to meet local public health needs, and provision of those services required, with appropriate training and support.
- Travel to offsite locations may be required, including to the Jefferson County Detention Center and community organizations.
- Other duties as assigned.
Qualifications: Minimum Qualifications:
- Master's degree in nursing from an accredited institution;
- Colorado Advance Practice Registered Nurse License with full prescriptive authority;
- 1 year of work-related experience; and,
- Current CPR certification at time of hire.
Preferred Qualifications:
- 3 years of work-related experience.
- Strongly prefer experience in public health, including family planning and HIV and STI prevention and management.
- Experience drafting policies and protocols.
- Spanish speaking ability. JCPH has a policy to additionally compensate workforce members who can demonstrate bilingual language skills.
Knowledge, Skills and Abilities:
- Knowledge of evidence-based practice and solid clinical decision-making skills.
- Taking histories performing physical exams, formulating assessments, and determining treatment plans for patients.
- Orders appropriate diagnostic studies and treatments plans, including prescribing medication.
- Ability to respond appropriately in emergency situations.
- Ability to learn and utilize an EHR to accurately document and report.
- Ability to establish and maintain positive and effective working relationships.
- Proficiency in Microsoft Office Suite is required.
