Job Title: Clinical Social Worker Senior Job Summary This position is responsible for performing social work assessments and interventions as needed for inpatients, outpatients and emergency department patients. They are dedicated to patient and family centered care that values personal self-determination, skilled at managing multiple priorities at once, and engaging in creative, compassionate and ethical problem-solving. As a member of a multidisciplinary health team, they perform comprehensive assessments and develop treatment plans that integrate the medical, social and resource issues that impact individual patients, families and at-risk populations. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plan of care for both individual patients and the medical community. Provides leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned social work staff. \nEssential Functions (Key Roles & Responsibilities) \n\n1. P\nrovides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues. \nFormulates biopsychosocial assessment, disposition and treatment plans. \n\n2. P\nrovides mandated assessments when abuse is suspected (child, disabled adult, elder) and specialized safety assessment when violence is suspected Plan discharge from care facility to home or other care facility. \n\n3. C\nollaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. \n\n4. M\nonitor, evaluate, and record client progress according to measurable goals described in treatment and care plan. \n\n5. S\ncreen, address, and manage social determinants of health \n\n6. P\nrepares and maintains all pertinent records, statistics, and progress notes. \nCompetencies/Knowledge, Skills & Abilities \n\n1. D\nemonstrates a mastery of a range of specialized clinical diagnoses and interventions. \n\n2. C\nonducts clinically competent complex and high-risk assessments, treatment plans and interventions. \n\n3. D\nemonstrates mastery in problem solving abilities, conflict resolution, efficiency, and communication skills. \n\n4. F\normulates advanced psychosocial assessments and treatment plans. \n\n5. S\nupports other staff in developing assessments and plans in complex cases. \n\n6. D\nemonstrates advanced clinical judgment and innovation in advocating for clients. \n\n7. D\nemonstrates advanced working knowledge of internal and external resources and refers appropriately. \n\n8. D\nemonstrate ability to identify problems in order of urgency and initiate treatment plans towards timely solutions and appropriate time management skills. \n\n9. D\nemonstrates the ability to articulate SW clinical perspective in team meetings, effectively collaborates with the interdisciplinary team to comprehensively problem solve and optimize a plan of care. \n\n10. E\nscalates to Social Work leadership all operational issues and high risk and complex cases. \n\n11. M\nay provide clinical supervision based on department need (MSW students and/or social work staff). \n\n12. S\neeks opportunities in the department and hospital or may be asked by SW leadership to participate in initiatives, projects, quality reviews, and committees and participates in these at an entity and Enterprise level. \n\n13.
