Job Overview
We are seeking a dedicated and skilled Nurse Practitioner to join StrideMD Telehealth team. The ideal candidate will be responsible for providing high-quality patient care, diagnosing and treating our Diabetes Patients, and collaborating with other healthcare professionals to deliver comprehensive care. This role is essential in ensuring patient health and wellness, particularly in areas such as infectious disease care and internal medicine.

Patient Assessment and Management:

• Conduct thorough assessments of patients with diabetes, including medical history, physical exams, and laboratory tests.
• Develop, implement, and monitor individualized care plans to manage diabetes, in collaboration with physicians and healthcare teams.
• Adjust and prescribe diabetes medications and insulin based on clinical guidelines and patient response.
• Provide telemedicine consultations to patients, offering remote management and follow-up for patients who require virtual care.

Telemedicine Care:

• Deliver diabetes care and follow-up through telemedicine platforms, ensuring access to care for remote or home-bound patients.
• Educate patients on how to use telemedicine tools, such as continuous glucose monitors (CGMs) and remote patient monitoring devices.
• Maintain regular communication with patients through virtual visits to monitor their progress, assess complications, and adjust treatment plans.

Patient Education:

• Educate patients and their families about diabetes management, including nutrition, exercise, and lifestyle modifications.
• Provide instruction on blood glucose monitoring, insulin administration, and proper medication adherence.

Monitoring and Follow-up:

• Monitor patients’ blood glucose levels, complications, and overall health status, adjusting care plans as needed.
• Conduct routine follow-ups with patients to track progress, address concerns, and ensure adherence to treatment plans.
• Manage the initiation and adjustment of insulin pumps or continuous glucose monitoring devices (CGM).

Collaboration:

• Work collaboratively with physicians, nurses, dietitians, and other healthcare providers to deliver integrated care.
• Act as a liaison between patients and the healthcare team, ensuring clear communication and understanding of care plans.

Documentation and Reporting:

• Maintain accurate and up-to-date patient records, including treatment plans, assessments, and follow-ups.
• Document telemedicine consultations according to regulatory standards, ensuring compliance with telehealth best practices.
• Provide reports on patient progress and outcomes to the supervising physician and healthcare team.

Professional Development and Education:

• Stay current on best practices, new medications, and emerging technologies in diabetes care through continuing education.
• Educate and mentor junior staff and colleagues in diabetes management protocols, patient care strategies, and telemedicine practices.

Qualifications:

• Master’s degree or higher in nursing (Nurse Practitioner) or physician assistant studies.
• Current licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in [Insert State/Region].
• Certification in diabetes care and education (e.g., CDCES – Certified Diabetes Care and Education Specialist) Required.
• Minimum 2 years of clinical experience, with at least 1 years specializing in diabetes care.
• Experience in delivering care via telemedicine platforms is required.
• Strong knowledge of diabetes medications, insulin management, and glucose monitoring technologies.

Skills and Competencies:

• Proficiency in the use of telemedicine technologies, including video conferencing, remote patient monitoring devices, and secure messaging platforms.
• Excellent patient communication and education skills.
• Ability to work collaboratively in a multidisciplinary team environment.
• Proficient in the use of electronic health records (EHR) for documentation and patient management.
• Strong organizational and time management skills.
• Compassionate and patient-centered approach to care.

Working Conditions:

• Full-time position, with occasional evening or weekend shifts depending on patient needs.
• Combination of in-office and telemedicine work. May involve some travel for patient home visits or community education programs.
• Telehealth responsibilities will require access to a private, secure location for remote patient consultations.
