The Care Promoter 1, Early Embrace is an unlicensed medical professional cross-trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties of Care Promoter 1 vary from setting to setting, depending on the size, location and type. In addition to administrative duties, the incumbent in this role perform clinical duties under direct physician supervision and in accordance with state medical practice acts. The Care Promoter 1, Early Embrace is an integral member of a Physician-lead Care Team that is focused on providing excellent and comprehensive primary care for a specific population of patients
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Hybrid role supporting PCPs for patient care and managing day to day center operations ensuring that today's work is done today with special attention to CMAP and HEDIS measures.
Supports smooth operations with special attention to scheduling, patient wait times, and other areas of concern to increase efficient and quality services.
Assists the Center Clinical Manager in the supervision of Care Promoters and other support staff, including input for performance reviews and corrective action plans as needed.
Under the direction of the Center Clinical Manager oversee and train new and existing staff Care Promoters on health education, laboratory, and Medication Room services.
Prepares patients for examinations and performs routine screening tests; assists physician with exams; explains treatment procedures and physicians' instructions to patient.
Processes patient phone messages, returning calls and routing them to other team members as appropriate; calls patients to obtain and relay pertinent information for the physician.
Upon physician approval and authorization, calls in prescriptions to the pharmacy. Uses communication skills with appropriate medical terminology; and follows appropriate legal and ethical professional conduct; authorized individuals will use the ChenMed Rx system to support physician medication dispensing.
Under the direction of the Center Clinical Manager oversees center quality activities including but not limited to: equipment checklists, monthly self assessments, waived testing quality control, inventory management
Organizes and participates in quality improvement activities that promote center safety and improved patient outcomes.
Performs other duties as assigned and/or delegated at manager's discretion.
KNOWLEDGE, SKILLS & ABILITIES: Knowledge of ChenMed Medical products, services, standards, policies and procedures
Knowledge and understanding of all applicable laws and regulations and ChenMed's philosophy, policies, procedures, and standards
Superior knowledge and skill in standard medical assistant intake procedures, including but not limited to: taking patient vitals and obtaining and documenting blood and other specimen samples for laboratory work
Superior leadership, clinical, conflict resolution, communication and problem solving skills
Excellent verbal communication skills
Good organizational and prioritization skills. Ability to remain calm in stressful situations
Ability and willingness to provide extraordinary customer service in all patient, customer and family member interactions.
Ability to operate successfully in a fast-paced, ever-changing environment
Astute ability to maintain professional conduct and confidentiality in the care of patients
Ability to effectively communicate and work with doctors, patients and others in a professional and courteous manner
Ability to pay close attention to detail and to ensure accuracy of reports and data
Keen ability to manage multiple processes and effectively lead a team of Care Promoters and other clinical staff
Proficient in Microsoft Office Suite products including Word, Excel, PowerPoint and Outlook, database, and presentation software
Ability and willingness to travel locally, regionally and nationwide up to 20% of the time
Spoken and written fluency in English required, bilingual a plus
EDUCATION AND EXPERIENCE CRITERIA: High school diploma or equivalent education (GED) required
Graduation from a nationally accredited Medical Assistant program OR comparable healthcare vocational training (i.e., foreign-trained physician, paramedic, etc.) required
A minimum of 1 year of work experience as a Medical Assistant, Patient Care Technician or similar in a hospital, medical clinic or comparable environment OR documented clinical rotation training can be substituted for work experience
Experience working with geriatric patients is a plus
EMR system experience preferred
Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
